Stefanie Holmes

Officer Manager

Stefanie was born and raised in Salem County.  She became a member of the PRAC staff in 1986 were she worked as an administrative assistant. From 1990 to 1993 she took her talents to the Salem County Office on Aging.

Her passion with serving the community led her back to PRAC where she has held a multitude of administrative and supervisory positions over the last twenty five years. She currently is the Office Manager of the organization overseeing the day to day operations of all the administrative functions of the agency to include personnel, purchasing, human resource and assists with program delivery.

She has been the consummate professional throughout the years while at PRAC. As PRAC continues to grow its programs and region of service according to the Board and C.E.O. she is the “boots on the ground go to person.” Stefanie resides in Pittsgrove Township with her husband and daughter.

Joceline Ortiz

Outreach Coordinator for Breast Health Awareness

Joceline has an Associate’s degree in Social Services from Cumberland County College. She is currently working towards her Bachelor’s degree in Psychology at Wilmington University.

Joceline became a member of the PRAC staff in 2014 where she first worked part-time as a caseworker for their multi-service department. She later took a full-time position working for the Senior Connect program. Joceline then transitioned to the LIHEAP program as a caseworker to assist clients with heating and electrical needs.

Currently, Joceline is the outreach coordinator for Latina Breast Health Awareness. Her responsibilities are to develop, organize, and produce the breast cancer health educational materials needed for community presentations, classes, and activities. She also recruits breast health education class participants through community outreach.

Joceline is passionate about empowering women throughout the communities and teaching them the importance of mammograms and self-care.

In her free time, Joceline continues to empower and teach others at Praise in Action ministries, a school for the worship arts that she co-founded.

Ernesto Ortiz

Housing Engagement Officer

Ernesto (Rick) Ortiz received an Associates of Applied Science degree in Social Services from Cumberland County and received his Bachelor’s in Psychology from Wilmington University. Rick is currently working on his Master’s degree in Clinical Social Work.    

After working 12 years in the public sector Rick started working with a local non-profit organization as a Director for Children and Youth programs. In 2016 Rick accepted a full time position with PRAC of Southern New Jersey as a Health and Wellness promoter in partnership with the Salem County Health and Wellness Foundation. In 2017 Rick became the PRAC Housing Engagement Officer in partnership with the Cumberland County Housing First Collaborative founded by the M25 Initiative working to end chronic homeless in Cumberland County NJ. Currently Rick continues to work as the Housing Engagement Officer and the Non-Profits Code Blue Coordinator assisting Code Blue with operations and holistic case management during Code Blue activation’s.  

 Rick continues to empower and teach others at a local church as Pastor of Praise in Action Ministries. Rick also works together with the Millville Police Chaplain Program with the local law enforcement officers.

Geovanni Cantizano

Program manager

Geovanni Cantizano has been active in the field of human services for 25+ years, focusing on assisting individuals and families reach their full potential through one on one and group counseling, workforce development, soft skills training and program management in New York, New Jersey, Pennsylvania, Delaware, Maryland and Washington, D.C.

Geovanni has managed program and operations for institutions such as ResCare (the largest for profit human services provider in the country), Maximus, Inc., Northern Manhattan Improvement Corporation and Ft. George Community Enrichment Center (Headstart).  In addition, he has three years of experience as a counselor for victims of domestic violence and victim of crimes in general.

He currently serves as program manager for PRAC of Southern NJ, Inc., anddelivers services through the health and wellness program, Residential Maintenance Program, Assisted Transportation Program, and the Housekeeping Program throughout Salem County, NJ.
